Assumption of Risk & Release of Liability
I, the undersigned parent or legal guardian of the above-named minor, acknowledge and agree to the following on behalf of myself and my child:
- Assumption of Risk. I understand that participation in baseball tryouts involves inherent risks of physical injury, including but not limited to sprains, fractures, concussions, and other injuries that may result from contact with equipment, other participants, or the playing surface. I voluntarily accept these risks on behalf of my child.
- Release of Liability. In consideration of my child's participation, I hereby release, waive, and discharge Machine Athletics Association, its directors, officers, coaches, volunteers, and agents from any and all claims, demands, or causes of action arising out of or relating to my child's participation in this tryout event, to the fullest extent permitted by Ohio law. I understand this release does not apply to claims arising from gross negligence or willful misconduct.
- Physical Fitness. I confirm that my child is in good physical health and has no known medical condition that would prevent safe participation in athletic activity. If my child has any physical limitations, I accept full responsibility for informing the coaching staff prior to the tryout.
- Emergency Medical Authorization. In the event of a medical emergency, I authorize Machine Athletics Association staff to seek and consent to necessary emergency medical treatment for my child. I understand that MAA is not responsible for any costs associated with such treatment.
- Photo & Media Release. I grant Machine Athletics Association permission to photograph and/or video record my child during tryout activities, and to use such images for promotional, educational, and social media purposes without compensation.
- Code of Conduct. I agree that my child and I will conduct ourselves in a respectful and sportsmanlike manner throughout the tryout. Machine Athletics Association reserves the right to remove any participant or family member who engages in disruptive, unsafe, or unsportsmanlike behavior.
This waiver is governed by the laws of the State of Ohio. If any provision is found unenforceable, all remaining provisions remain in full effect. This document constitutes the entire agreement between the parties regarding assumption of risk and release of liability for this event.
